Hypertension almost never hurts, which is exactly why it's dangerous. Here's a plain-English guide to what your two numbers actually mean, how to measure them properly, and how we bring them down — safely and for good.

If a blood-pressure reading has just landed you here, start with this: high blood pressure is the most common serious condition I treat, and one of the most fixable. It rarely makes you feel sick — which is precisely why it does so much damage before anyone notices. The good news is that we understand it deeply, the medications are cheap and effective, and most people can get to a safe number without upending their lives.

Why they call it the "silent killer"

Most conditions announce themselves. Hypertension doesn't. You can walk around for years with a pressure of 160/100 feeling completely fine while, quietly, the extra force is wearing down the very system it flows through. That's the whole problem: there's usually nothing to feel. The headaches and nosebleeds people associate with high blood pressure are mostly myth — by the time symptoms show up, the numbers are often frighteningly high.

What that steady pressure does is mechanical. Blood vessels are meant to flex; under chronic high pressure they stiffen and scar, translating over time into real, nameable harm:

What the two numbers actually mean

Blood pressure is written as a fraction — say, 128/78 — because it captures two moments in every heartbeat. The top number, systolic, is the pressure when your heart squeezes and pushes blood out. The bottom number, diastolic, is the pressure resting between beats, when the heart refills. Both matter, though in adults over fifty the systolic number tends to be the one that drives risk.

For most adults, the target we treat toward is under 130/80. That's a meaningful shift from the old "140/90 is fine" thinking — the data on preventing strokes and heart attacks at the tighter goal is strong. But, and this is the part that matters, the right target is individualized. For a frail, older patient prone to dizziness and falls, chasing 130/80 can do more harm than the number prevents; a slightly higher, steadier goal is often smarter. As with most of medicine, the best target is the one you can reach and hold safely.

One reading almost never earns a diagnosis in my office. Blood pressure is a moving thing — it climbs when you're rushed, in pain, or sitting in a paper gown in a cold room. I'd rather see a week of calm readings from your own kitchen table than one anxious number from mine. When someone shows me a log instead of a single snapshot, we make far better decisions together — and I've talked plenty of people out of a lifetime of pills that a proper measurement showed they never needed.

Measuring at home — and why it beats the office

If you take one practical thing from this article, make it this: a good home blood-pressure monitor is one of the highest-value purchases in all of medicine. Office readings mislead in two opposite directions. Some people spike in the clinic and are normal everywhere else — white-coat hypertension — and get treated for a problem they don't have. Others read fine in the office but run high the rest of the day — masked hypertension — and get falsely reassured while damage accumulates. Home readings catch both.

But the readings are only as good as the technique. To measure well:

People are often surprised how much blood pressure responds to how they live — some of these changes rival a medication. The ones that actually move the needle:

Cut the sodium — and add potassium

Sodium is the biggest dietary lever. The DASH eating pattern — rich in vegetables, fruit, whole grains, and low-fat dairy, low in salt and processed food — can lower systolic pressure by roughly 8–14 points, comparable to a first medication. Most of the salt we eat isn't from the shaker; it's hidden in bread, restaurant food, canned goods, and processed snacks. On the flip side, potassium (leafy greens, beans, bananas, potatoes) helps the body shed sodium and lowers pressure — though anyone with kidney disease should raise potassium only with their physician.

Medications — and why most people need more than one

When lifestyle isn't enough — and often it isn't, especially in stage 2 — medication is safe, inexpensive, and effective. The modern approach follows a principle that surprises people: we'd rather use two medications at low doses than one at its maximum. Different classes attack the problem from different angles, so combining them works better, with fewer side effects, than pushing any single one to its limit. Most patients ultimately need two or three, and that's not a failure — it's just how the physiology works.

When someone needs a third or fourth medication and still isn't at goal, my instinct isn't to keep piling on pills — it's to step back and ask what I'm missing. Are they actually taking the medications? Is there hidden salt, too much alcohol, undiagnosed sleep apnea, or a pain reliever quietly working against us? Is the cuff even the right size? Nine times out of ten, "stubborn" blood pressure turns out to have a reason once you go looking for it. The tenth time points to a specific hidden cause worth testing for — and finding it changes everything.

When blood pressure won't come down: resistant and secondary causes

We call it resistant hypertension when pressure stays above goal despite three well-chosen medications (one of them a diuretic) at good doses. Before labeling it that, we rule out the common imposters — a cuff that's too small, medications not actually taken, and hidden salt or alcohol. When it's genuinely resistant, adding spironolactone is often remarkably effective.

Resistant or unusually early or severe hypertension also prompts a search for a secondary cause — a specific, sometimes correctable driver hiding underneath. The ones worth checking include primary aldosteronism (more common than long assumed), obstructive sleep apnea, kidney disease, narrowing of the kidney arteries, thyroid disorders, and certain medications (NSAIDs, decongestants, some birth control). Finding one of these can occasionally cure the hypertension outright rather than just manage it.

When it's an emergency — call 911

A single high reading is usually not a crisis. But a very high pressure (roughly 180/120 or above) combined with symptoms is an emergency, because it can mean active damage is underway. Call 911 — don't drive yourself — if a very high reading comes with:
