If a lab result just told you your cholesterol is high, here's the most reassuring thing to know first: a high number on a page is not a diagnosis, and it is not a verdict. Cholesterol only matters in the context of everything else about your heart. The real question is never "is this number high?" β€” it's "how much does this number raise your risk of a heart attack or stroke, and what's the smartest way to lower it?" That's a very answerable question.

What cholesterol actually is β€” and why you need it

Cholesterol has a terrible reputation for something your body cannot live without. It's a waxy fat that every cell uses to build its outer membrane, and it's the raw material for vitamin D, bile acids that digest your food, and hormones like testosterone, estrogen, and cortisol. Your liver makes most of it; only a smaller share comes from your diet. So the goal is never to eliminate cholesterol β€” it's to keep the wrong kind from building up where it does damage: inside the walls of your arteries.

Because cholesterol is a fat, it can't dissolve in blood on its own. It travels wrapped in protein packages called lipoproteins, and the whole story of "good" and "bad" cholesterol is really a story about which package it's riding in.

LDL β€” the number that actually causes plaque

LDL (low-density lipoprotein) is the "bad" cholesterol, and here the old clichΓ© happens to be right β€” but for a reason worth understanding. LDL particles carry cholesterol out to the tissues, and when there are too many of them circulating, they burrow into the artery wall, get trapped, and trigger inflammation. Over years this builds the fatty, gritty deposits we call plaque. When a plaque cracks open, a clot forms on top of it β€” and that's a heart attack or stroke. LDL isn't just a marker that travels with heart disease; decades of genetic and drug-trial evidence show it's a direct causal driver. Lower the LDL, and you literally slow β€” sometimes reverse β€” the process. That single fact is the backbone of nearly everything we do about cholesterol.

HDL β€” the cleanup crew

HDL (high-density lipoprotein) is the "good" cholesterol; it ferries excess cholesterol back to the liver for disposal, so higher levels generally track with lower risk. But there's a modern caveat: trials that artificially raised HDL with medication did not reduce heart attacks. HDL is a useful signal of overall metabolic health, not a lever we push directly. A low HDL tells me to look harder at the whole picture; it isn't something we treat with a pill.

Triglycerides β€” the other fat

Triglycerides are the storage form of fat in your blood, and they climb with excess sugar and refined carbs, alcohol, extra weight, and poorly controlled diabetes. Mild elevations add to cardiovascular risk; very high levels (roughly above 500 mg/dL) become an urgent, separate problem because they can inflame the pancreas. The good news is that triglycerides respond dramatically to lifestyle β€” often the most movable number on the whole panel.

The shift that changed everything: we treat risk, not a number

When the picture is genuinely borderline and a patient is on the fence about starting medication, a coronary artery calcium (CAC) score β€” a quick, low-dose CT that literally counts the calcified plaque in your heart's arteries β€” can be the tie-breaker. A calcium score of zero is genuinely reassuring; a high one reveals disease that's already underway and often changes the plan on the spot.

The visit that sticks with me most is the patient who arrives clutching a printout, terrified because one line is flagged in red. Almost always my first move is to put that number down and ask about everything around it β€” age, blood pressure, whether anyone in the family had an early heart attack, whether they smoke. Sometimes we calculate the risk together on the screen and the fear eases immediately, because the honest answer is "your ten-year risk is low, and we can watch this." Other times the number is modest but the surrounding risk is real, and we start treatment. The lab value alone almost never decides it.

Statins β€” still the foundation, and worth an honest conversation

When medication is the right call, statins are almost always where we start, and for good reason: no class of cardiovascular drug has more evidence behind it. Statins block the enzyme your liver uses to make cholesterol, which prompts the liver to pull more LDL out of the bloodstream. The result isn't just a lower number β€” it's fewer heart attacks, fewer strokes, and longer lives, proven across tens of thousands of patients over decades. For the right patient, the benefit is large, durable, and about as well established as anything in medicine.

And yet no drug generates more hesitation, so let me be straight about the side effects. The one people worry about most is muscle aches. Real statin muscle symptoms do happen β€” but careful blinded studies have shown that the great majority of aches blamed on statins occur just as often on a placebo. This is the nocebo effect: when we expect a symptom, we genuinely feel it. That doesn't mean your aches aren't real or that I dismiss them β€” it means we can almost always solve them, by switching to a different statin, lowering the dose, trying every-other-day dosing, or briefly stopping and rechallenging to see what's truly the drug. Serious muscle injury is rare. A modest, manageable uptick in blood sugar can occur, and the old fear about liver damage has largely faded β€” routine liver monitoring is no longer even recommended. For a patient whose risk warrants it, walking away from a statin over an overblown worry trades a small, fixable inconvenience for a real, preventable heart attack.

When someone tells me they "can't tolerate statins," I've learned not to accept that as the end of the conversation β€” I treat it as a puzzle we haven't finished solving. More often than not we find a version that works: a different molecule, a smaller dose, or a slower on-ramp. I'd rather spend three visits getting the fit right than write someone off, because the patients who quietly stop their statin and never mention it are the ones I worry about years later. The goal isn't to win an argument about side effects β€” it's to find the plan you'll actually stay on.

When a statin isn't enough β€” the newer add-ons

Diet and lifestyle β€” real, essential, and usually not the whole answer
