1) What telemedicine offerings do you offer?
We plan to implement the following telemedicine services:
Virtual Consultations : Patients can connect with our Physician from the comfort of their homes through a HIPAA-compliant online video conferencing module. These consultations cover detailed management plans for conditions such as hypertension, diabetes, sexually transmitted diseases, urinary tract infections, viral illnesses, obesity management, and more.
Remote Monitoring Services : For patients requiring ongoing health condition monitoring, we offer services utilizing wearable devices or other health tracking tools.
E-Prescriptions and Digital Health Records : We utilize an EMR system for e-prescriptions through our telemedicine platform.
Health and Wellness Webinars : To promote overall well-being, we host webinars led by our healthcare professionals. Participants can register and pay for these webinars through our platform, with Stripe handling secure payment transactions.
Telemedicine Membership Plans : We have membership plans for telemedicine services with various tiers based on the level of coverage and benefits. Patients can subscribe to these plans and make recurring payments through Stripe.
2 ) what payment platform do you use for online payment?
We utilize Stripe for online payment processing on our website using payment links and for payments made via phone. We do not foresee using platforms like Amazon or eBay to sell our services.
3) If you ’ re selling physical products, please provide details about your shipping licensing and/or agreements. If you don ’ t require a shipping license or agreements, please explain why.
Currently, we are not selling any physical products. Prescriptions are controlled by the Medical Board of California and the DEA (Drug Enforcement Administration).
4) How do you ensure that patients are only connected with physicians licensed to practice in the state of the patient?
Before patient evaluation, we confirm their home address/insurance using a legal document, such as a current State ID or a current driving license.
5) How do you ensure compliance with telemedicine regulations in each state you operate in?
We follow the State guidelines issued by the Medical Board of California. We obtain and document verbal or written consent from the patient for the use of telehealth as an acceptable mode of delivering services.

6) Are you HIPAA compliant?
Yes, we are HIPAA compliant.
We utilize and pay for a popular and successful platform for telemedicine visits.
7) Are consultations synchronous (via video or telephone) or asynchronous (via online questionnaire)?
Consultations are synchronous, with most information obtained live. Although EMR has a built in way of obtaining patient information with patient intake forms -filled out by the patient prior to the appointment.
8) Will you be adding more providers to your practice?
When any providers are added in the future, we will update all their information, on the web portal
9) On average, how long does it take for a patient to receive a consultation after they make a request?
Anywhere from 1-2 hours to 14 days.
10) If medicines are prescribed at the end of the consultation, please provide a brief description of how prescriptions are provided, and the patient ’ s options to obtain medication.
Built-in SureScripts e-prescribing platform to prescribe routine medications to the patient ’ s preferred pharmacy of choice (based on location or pricing). We use ECPS prescribing for other scheduled drugs.
11) In case you partner with a specific pharmacy to dispense medication, please provide details of this partnership, including the funds flow.
We have no partnerships with any pharmacy.
12) Do you provide your product and/or service directly, or are orders fulfilled by a separate supplier or vendor?
The evaluation and telemedicine services are provided directly to the patient. Most orders are filled at local pharmacies of their choice.
13) Please provide details on your refund policy.
Our telemedicine services maintain a strict no-refund policy for medical evaluations. Once a commitment is made and the evaluation is completed, refunds are not possible. If a payment is made and an evaluation is not executed within the stipulated 30-day timeframe, customers are entitled to request a refund equivalent to 50% of the fees, exclusive of payment processing charges(variable), credited back to their respective mode of payment accounts.
14) How do you monitor fraudulent activity?
We ensure that the integration process collects important information about the customers. We try and limit our exposure to disputes and fraud.
15 ) Whats your cash pay typical customer? What are the estimates on fees charged for cash only- direct patient care?
Average amount: $125-250
Fees vary depending on the type of appointment
Obesity evaluation: $150-$500
Hypertension management: $125
Thyroid consultation and management: $150
16) How do you plan on growing your customer base?
We rely mostly on organic growth based on word of mouth. we may add our services guide on social media as well.

In the event of a payment being rendered and a partial evaluation conducted, if a refund request is submitted within a 30-day period, the customer shall receive a reimbursement equivalent to 15% of the fees remitted, excluding payment processing charges. If a payment is made and an evaluation is not executed within the stipulated 30-day timeframe, customers are entitled to request a refund equivalent to 50% of the fees, exclusive of payment processing charges, credited back to their respective mode of payment accounts.
